Most crane incidents are not mechanical failures. They are an operator working from the wrong column of the right chart.
Mistake one: the wrong configuration
Charts are published per configuration — counterweight fitted, outrigger extension, jib offset, reeving. Read the chart for the configuration the machine is actually in, not the one it was in last week.
Mistake two: measuring radius from the wrong point
Radius is measured from the centre of rotation to the centre of the load, horizontally, with the load suspended. Not from the boom foot, and not from where the load is sitting on the ground before you pick it.
Mistake three: assuming on-outriggers capacity applies on tyres
Pick and carry duty is a different, much lower chart. A crane rated 20 tonnes on outriggers may be rated well under half that travelling on tyres.
The limiter is a backstop, not a plan
A rated capacity limiter protects against the last mistake, not the first. It cannot know that you selected the wrong chart.
